This position will require a TB Test or Chest X-Ray prior to start. Cost of the TB Test/Chest X-ray is completely covered by the Center.

GENERAL DESCRIPTION

Performs highly complex clinical services work. Work involves clinical screenings, assessments, and determinations for hospitalization and/or ongoing service eligibility based on consumer needs both in-office and within the community. Coordinates, links, and authorizes appropriate services and monitors consumer progress with meeting stabilization goals. Works under limited supervision with considerable latitude for the use of initiative and independent judgment.

This position provides a majority of services in the community, such as homes, jail settings, shelters, hospitals, and other environments in which the client lives. Due to this, this position is required to travel frequently within the service area and to interact frequently with clients, families, law enforcement/justice system officials, hospital staff, and other community stakeholders involved in the recovery of the individual they are serving.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Assists the Crisis Director in quality assurance to ensure compliance of crisis programming.
  • Assist the Crisis Director evaluate team activities and provide feedback to further individual and team development.
  • Assists the Crisis Director in executing action plans to increase efficacy within crisis services.
  • Attends community meetings addressing mental health services and programs.
  • Provides consultation to internal and external stakeholders regarding crisis responses.
  • Conducts crisis assessments and services within required timelines per the Texas Administrative Code.
  • Completes all documentation in the EHR according to Texas Administrative Code, contract and Center requirements.
  • Serve as a linkage into outpatient care, to include facilitating intake interview, coordination with LPHA for diagnostic recommendation, development of person-centered recovery plan, and referral to indicated services.
  • Provide hospital discharge follow up within seven days, crisis follow up and transition services to individuals.
  • Identifies problem areas, service gaps, and needs that are barriers to accessing or participating in services by assessing individuals' support systems and available community resources, schools, transportation, etc., and making necessary referrals and plans to meet identified needs and outcomes.
  • Participates in Random Moment Time Study/Medicaid Administrative Claiming (MAC) as required.
  • Comply with the Center's Work Values and Behavior Addendum.
  • Regular attendance, dependability, and promptness are required for the scheduled workday or agreed upon hours 100% of the time, to ensure consistency and completeness of program's processes.
  • Professional Development - Successfully complete and stay compliant with training required to maintain skill competency and cross training to ensure consistency across program procedures.

GENERAL QUALIFICATIONS
Experience/Education/Licensing/Certification
  • Experience in a field relating to the agency program.
  • Graduation from an accredited four-year college or university with major course work in a field relevant to the assignment.
Knowledge/Skills/Abilities

  • Ability to proficiently utilize computer programs, e.g., Microsoft Word and Excel.
  • Ability to effectively handle potentially aggressive behavior in a positive manner.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to present a professional appearance and conduct when representing the Center.

Additional Requirements
  • As frequent daily driving within the catchment area is required, must have and maintain a valid drivers' license with an acceptable driving record.
  • Pursuant to Texas Health and Human Services Performance Contract Information Item V, incumbent hired must provide proof of a negative TB test within two weeks of employment and annually thereafter.
