Coryell Health's Home Health is dedicated to providing exceptional home health care services to our patients. We specialize in delivering skilled nursing care on an intermittent basis in the comfort of our patients' homes. Our compassionate team collaborates closely with physicians and caregivers to ensure that each patient receives personalized and effective health care tailored to their needs.

Key Responsibilities:

• Deliver skilled nursing care to patients in their homes, following physician orders and individualized care plans.
• Perform assessments, monitor patient conditions, and document care provided in accordance with Agency policies.
• Assist patients with activities of daily living, offering private duty nursing care when necessary.
• Collaborate with the Registered Nurse to ensure continuity of care and participate in care planning.
• Educate patients and families on health management and treatment plans.

• Graduate of an approved school of vocational nursing; current Texas Licensure required.
• Minimum of one year of nursing experience preferred, particularly in home health or similar settings.
• Strong understanding and acceptance of the philosophy and goals of the Agency.
• Effective communication skills and ability to work both independently and collaboratively within a team.
• Capable of exercising initiative and independent judgment in patient care situations.
