Key Responsibilities:

• Provide safe transportation for residents and hospital patients to medical appointments and other designated destinations.
• Assist in executing both group and individual activities as planned by the Activities and Transportation Coordinator.
• Foster resident engagement by encouraging participation in various social and recreational activities.
• Maintain and update the Transportation Calendar with all scheduled appointments and gather/distribute resident mail.
• Observe and report any significant changes in resident attendance, mood, or involvement to nursing staff.

• High school diploma or equivalent; Certified Nurse's Aide Certification preferred.
• Current CPR certification and a clean driving record.
• Proficient in computer skills for documentation and communication purposes.
• Excellent verbal and written communication skills with the ability to multitask effectively.
• Physical capability to assist residents, including lifting items up to 35 pounds and pushing wheelchairs.
