Heart to Heart Hospice - - Responsibilities: Provide direct patient care to patients under the direction of the Registered Nurse and according to the Hospice Aide/Homemaker Plan of Care; Assist with safe transfers and ambulation per agency policy; Report and document changes and care provided to ensure continuity of care; Maintain infection control practices and standards; Communicate with patient, family, and staff in a respectful and supportive manner
